2) Procedures for Requests for Disclosure, etc.
1. After receiving the application, we will send the prescribed request form "Request for Disclosure of Personal Information, etc. Subject to Disclosure" to be used by us by post.
2. Please post the completed invoice, the document confirming that you are an agent if requested by an agent, and the postal money order for the service charge (only in the case of notice of purpose of use and request for disclosure) to the above person in charge of personal information inquiry.
3. After receiving the above invoice, in order to confirm the identity of the customer, we will inquire about two items of the personal information registered with us that can be confirmed by the customer (e.g. telephone number and date of birth, etc.).
4. In principle, the response shall be made in writing (by sealed mail) to the person in question.
